Pathogenesis and exploration of immunotherapeutic approaches in type-1 diabetes (T1D)

·       Manipulation of preproinsulin-specific regulatory T cells for therapeutic approaches in T1D

Combining the approaches of identification of epitope-specific regulatory T cells (Tregs) using MHC multimers and Treg specific markers can facilitate the scope of antigen-specific immunotherapy in humans. Animal studies have indeed shown that islet antigen-specific Tregs can indeed reverse T1D by targeting T cells that infiltrate pancreas. Our lab is involved in generating and manipulating preproinsulin-specific Tregs from natural and induced Tregs and assessing their specificity, stability and suppressive ability to inhibit beta cell-specific effector T cells to prevent T1D in vitro and in vivo.

 

·       Innate immune mechanisms involved in initiation and pathogenesis of T1D

The initial pathogenic mechanisms involved in the cross-talk of innate immune cells and beta cells during initiation of T1D are far from clear. Our current study is focused to elucidate the role of plasmacytoid dendritic cells (pDCs) and monocytes in initiation of T1D via self-nucleic acid complexes. The study is investigating the ability of pDCs and monocytes to capture DNA/RNA complexes released during beta-cell death, and whether the uptake of self-nucleic acid complexes influences presentation of autoantigens to activate T cells and finally, whether these activated pDCs and monocytes are capable of inducing beta-cell apoptosis.

 

·       Suppression of type-1 diabetes in NOD mice by induction of colonic regulatory T cells

Defects in colonic regulatory T cells (cTregs) have been found in subjects with T1D. Butyrate, a postbiotic, is shown to favor induction of cTregs via inhibition of histone deacetylase on the Foxp3 gene as well as through induction of tolerogenic dendritic cells and macrophages. We have reported the immunomodulatory effects of butyrate in NOD mice via induction of cTregs that can migrate to the pancreas and pancreatic lymph nodes. We now plan to further extend this study in human subjects to assess the therapeutic benefits of butyrate in suppression of T1D.

·       Suppression of type-1 diabetes in NOD mice by induction of colonic regulatory T cells

It has been observed that the development of clinical diabetes is preceded by intestinal alterations such asan aberrant intestinal microbiota, a leaky intestinal mucosal barrier and an altered intestinal immune responsiveness. Defects in colonic regulatory T cells (cTregs) have been found in subjects with type-1 diabetes. Butyrate, a postbiotic, is shown to favor induction and activation of cTregs via inhibition of histone deacetylase on the Foxp3gene as well as through induction of tolerogenic dendritic cells and macrophages. Through this study we plan to explore the immunomodulatory effects of butyrate in induction of cTregs and further assess their ability to migrate to pancreas and suppress type-1 diabetes in NOD mice.
